PACT LES stands for Planning All Care Together Local Enhanced Service Agreement. The PACT LES is a way that local commissioners can encourage and incentivise GPs to focus more time on particular cohorts of patients – and Carers are one of the cohorts being targeted in the PACT LES 2013/14.
GPs in Wandsworth are now providing an annual 30-minute Carer Consultation for Carers on their Carers Register.
In the Carer consultation the GP is expected to collect and record information about the caring role, do a health check, consider the support a Carer may need, and give information, refer or signpost to any to further services that the Carer needs. As well as doing Carer consultations, practices also have to have a named GP Carer Lead and update and maintain their Carers Register. Please help your GP practice to help you by making sure that they know you are a Carer and checking that your name is on their Carers Register.
