Do you support someone who takes warfarin, or do you take warfarin yourself?

Wandsworth CCG (Clinical Commissioning Group) wants to hear from people who take warfarin and their Carers, to help improve warfarin services.  This is whether you get treatment from your GP or vsit a hospital.

We know that sometimes Cares can find waiting for blood results frustrating, and also travelling to the hospital for the blood test.  We would really like to hear from you, so we can make sure any changes we make to the service take into account your experiences and ideas for improvements.  We have devised a short survey, and would really appreciate it if you could take the time to complete it for us.  Of course, this can be anonymous, and you needn’t give any information about your personal circumstances or health.

To find out more and request your copy of the survey, please contact Kwan Ku at Kwan.ku’wandsworthccg.nhs.uk or call 020 8812 4037.  You can complete the survey on paper or over the phone with Kwan if you prefer.
