Transition Support Porject

Do you care for someone with a learning disability or dementia?  Are you planning for the future, considering housing options for the person you care for but not sure where to start? 

 

Then Wandsworth Carers’ Centre can support you.  The term transition used here refers to a change in living arrangements. Carers can find it challenging when planning for the future of the person they care for.  Ensuring Carers feel listened to and not isolated, the project supports Carers by empowering them to make change manageable

 

The Transition Support project offers:

 

  • 1:1 support empowering you to make changes using the Carers Outcome Star
  • Support by accompanying you on visits to accommodation being considered
  • Peer to peer support from someone who has direct experience of the issues you are facing
  • Ongoing follow up support as and when required

New Member of Staff at Wandsworth Carers’ Centre

Hello, my name is Tracie Gleeson and I recently joined Wandsworth Carers’ Centre as the Transition Support & Development Officer.

For the purpose of this project, ‘transition’ refers to change in living arrangements.  I will be supporting Carers if the person they care for has Dementia or a learning disability and future housing or living options are being considered.

Two very different groups in very different situations, so support will be based on individual needs.  My role will be to provide 1:1 support for Carers, adopting tools to make change manageable, providing relevant information making the transition as positive as possible.
